    Tell us about your recent visit or hospital stay

    Complete a brief online questionnaire and tell us about your recent emergency room evaluation, observation stay, or hospitalization. If available, you can also provide discharge paperwork, medication lists, laboratory results, imaging reports, specialist recommendations, and other relevant information.

    Meet with an experienced medical provider

    We'll review what happened during your ER visit or hospital stay, discuss how you are feeling since discharge, review your current treatment plan and medications, and answer questions about your diagnosis, results, or discharge instructions.

    Make a plan for what comes next

    Depending on your situation, we may be able to help manage appropriate ongoing symptoms, review or adjust or refill  medications when appropriate, order follow-up laboratory testing or other studies when clinically indicated, review new results, and help you prepare for your primary care or specialist appointment.

Helping Bridge the Gap Between Discharge and Your Next Appointment

Emergency departments and hospitals are designed to evaluate and treat immediate or serious medical problems. Once you return home, however, there may still be symptoms to monitor, medications to manage, test results to understand, and follow-up care to arrange.

Many patients are instructed to see their primary care provider or a specialist shortly after discharge but cannot get an appointment right away. This service is designed to help provide appropriate medical follow-up during that gap.

A Follow-Up Visit May Be Helpful If You:

Start the Next Step Before Your Specialist Visit

Sometimes useful follow-up can begin while you are waiting for a specialist appointment.

We can review the evaluation already completed in the emergency room or hospital, look at available laboratory and imaging results, review the discharge plan, and determine whether additional follow-up may be appropriate before your specialist visit.

When medically appropriate, this may include follow-up laboratory testing, medication monitoring, symptom reassessment, or gathering additional clinical information.

This may allow you to arrive at your specialist appointment with more complete and current information available for review.

The exact evaluation, testing, and treatment will depend on your symptoms, previous results, medical history, discharge recommendations, and the clinical judgment of your provider.

Sorting Out Medication Changes After Discharge

Hospital and ER visits often result in medications being started, stopped, or changed. It can sometimes be difficult to know which medications you should be taking once you are back home.

During your follow-up visit, we can review your current medication list, compare it with your discharge instructions, discuss possible side effects or concerns, and help clarify your medication plan when appropriate.

When Telemedicine Is Not Appropriate

ER & Hospital Discharge Follow-Up is intended for patients who have already been evaluated and discharged and who are stable enough to remain at home.

This service does not replace emergency care or an in-person evaluation when one is needed.

If you develop severe or rapidly worsening symptoms, difficulty breathing, severe chest pain, stroke-like symptoms, loss of consciousness, uncontrolled bleeding, severe weakness, significant confusion, or another possible medical emergency, call 911 or return to the emergency department.

Some post-hospital conditions may also require an in-person examination, repeat imaging, procedures, or specialist evaluation. If that is the case, your OnCall provider will recommend the appropriate next step.

Would highly recommend Robyn to any family who needs kind, compassionate, responsive, and superior care. I feel heard, as a mother, which is sadly very rare when engaging with the healthcare system. Telehealth is so convenient when you have kids and you need someone in the medical field to hear your concerns, lay "eyes" on your child, and to share expert advice/concerns.
